As a courtesy to our patients, we routinely file claims for payment of our services with the various insurance companies. However, the patients are ultimately responsible for the payment for the services they receive.
Many insurance companies and managed care plans require patients to request authorization from their carriers prior to being examined by our doctors. Please read your insurance card or booklet to familiarize yourself with your specific requirements. In some cases, your family physician or primary care physician is required to authorize your visit by issuing you a referral form. Finally, managed care plans routinely require prior certification before a surgery can be performed. Our philosophy is to assist you with these complicated procedures.
When an insurance company pays a claim, it issues an EOB that is sent to the patient. This clarifies what is eligible to be paid to a physician provider under the terms of your insurance contract. In most instances, the patient is required to pay a portion of the physician's fee; we expect that payment to be at or shortly after the delivery of service.
