To serve you in the best way possible, we work by appointment. We suggest that you call for a routine appointment four to six weeks prior to the time you wish to be seen. Always let us know of symptoms or issues that may indicate that you need to be seen sooner.

If you need to cancel an appointment, please do so as far in advance as possible. Please bring all insurance information, including insurance cards, and, if necessary, your HMO referral.

Every effort is made to see you at your appointment date and time but occasional emergencies or events may cause delays. If that happens, we will keep you informed and let you know of other options to fit your needs. Always feel free to call ahead to check for any known delays before coming in for your appointment. We look forward to serving you.

Payment in full is expected at the time of your visit. We are a participating member of most area insurance plans and are happy to assist you with insurance claims. As a financial courtesy, we will wait for payment from your insurance company; however, any co-payment, deductible or fee for uncovered service, and any previous balance not paid by your insurance company is your responsibility at the time of service. If you are participating with any PPO or HMO insurance plan, you will be responsible for the co-pay associated with your plan.
