For our staff to best serve you, please review the information below ahead of your appointment. Please ask our office any questions you may have, including what paperwork you may be able to fill out ahead of your visit.
Appointments & Scheduling
Every effort is made to see you at your appointment date and time but occasional emergencies or events may cause delays. If that happens, we will keep you informed and let you know of other options to fit your needs. Always feel free to call ahead to check for any known delays before coming in for your appointment. We look forward to serving you.
Billing & Insurance
Payment in full is expected at the time of your visit. We are a participating member of most area insurance plans and are happy to assist you with insurance claims. As a financial courtesy, we will wait for payment from your insurance company; however, any co-payment, deductible or fee for uncovered service, and any previous balance not paid by your insurance company is your responsibility at the time of service. If you are participating with any PPO or HMO insurance plan, you will be responsible for the co-pay associated with your plan.
Medical Records — Prior to 2/1/2024
- Visit this link and select “Requestors.”
- Create a new account or if you have an existing account, log in to the MedRelease tool.

Next, select “Submit Request” to complete the HIPAA-Compliant Electronic Authorization form.
- Finally, when you complete the form click “Authorize Release” at the bottom of the page.
